Scheduling

To schedule your appointment call our office at (952) 926-1626, as found on your referral form. Please notify us if you are unable to keep your scheduled time.
Inform our office of any special needs or concerns at this time.

Day of Appointment

Bring the written referral form given to you by your doctor.
Payment is due at the time of service. You will be quoted a fee when you schedule your appointment.
As a service to you, we can provide a dental claim form that you may submit as a request for reimbursement. Please inform us upon check-in.
Please remove all jewelry from the neck up prior to your appointment.

When the records are fully processed by our office, they will be delivered to your referring doctor.
